IMPORTANT:  The surgery CLOSES at 6pm every evening. There will be access for patients with pre-booked evening appointments as part of our Enhanced Access commitment.
The Dispensary is open between 9am & 5pm only, please collect your medication within these hours. if this is a problem please contact us to make a prior arrangement . 
SICKNOTES: You can self-certificate for 7 days, if you need a note following 7 days and it is a problem that has been dealt with by the Doctor or hospital then please use the Contact us online request below.
 

If you have a respiratory problem and book a face to face appointment with a Doctor or Nurse, please wear a face covering when you attend the appointment. Thank you

If you request a Doctors appointment, we have asked our receptionists to ask about the reason for your call so they can book you an appointment with the most appropriate Health Professional and you will be offered either a telephone or face to face appointment. Do not attend the surgery unless you have an appointment arranged. 

ONLINE MEDICATION ORDERING  If you wish to write a message to dispensary when you order medication, please write it in the box on the second page, beneath the list of medication you have ordered, not the first page (as this deletes the list of medication you want). Thank you

Medication reviews. In future, on your repeat prescription list, if there is a message in bold print, please take note of it as it will be a message for you personally. (The automatic message has been removed). We are reviewing how medication reviews are carried out and you may note your review date may have been changed.

Online 'Pre-Registration' With The Practice

PreRegistrationIf you wish to pre-register with us, click on the links below to download and open the forms. Complete all details and bring the forms to the surgery when you visit for the first time. You will be asked to sign the forms to confirm that the details are correct.

We ask you to fill out a patient questionnaire. This is because it may take some time for us to receive your full medical records. 

If you wish to have online access to your records, make appointments and order repeat prescriptions, please complete the PatientOnline access form and bring 2 forms of ID when you register. One must be photographic and one as proof of address such as bank statement, council tax bill, utility bill. They must be separate documents.

ONLINE ACCESS TO PATIENT RECORDS from 31st October 2023

After 31st October 2023, all patients aged 16 and above, registered with a general practitioner will have the ability to automatically view information subsequently added to their electronic GP record – that is, information added on or after 1st November.

For patients who already have full online records access, nothing will change.

For patients who have “core” online access – that is, the ability to just request repeat medication, make appointments, and update demographic information – they will now be able to view new information added to their GP record – consultations, results, documents, free text etc. from 1st November onwards.

For patients who do not currently have online access, whenever they  sign up to the NHS App they will automatically be able to view information added to their GP record from 1st November 2023 onwards

For young people who have an NHS App login, they will automatically get independent record access once they turn 16. 

For new patients registering with the practice after 1st November, access will be given from the patient's registration date.
